Providence St. Peter Hospital is pleased to announce it has received the Washington Award of Excellence in Healthcare Quality from Qualis Health for its evidence-based care model to reduce readmissions of heart failure patients.  St. Peter is one of five hospitals in the state of Washington to receive an Excellence in Healthcare Quality Award.

This project is being recognized for its achievements in improving healthcare quality and outcomes because of its success in improving patient care through a dedicated team of pharmacists and heart failure nurse navigators who leverage their unique skillsets on every patient.

Preventing readmissions for heart failure patients is a strong focus of national quality improvement efforts. As a result of resources for the heart failure population, the Providence St. Peter Hospital transition-of-care team corrected medication errors before patients were discharged. Additionally, the average 30-day heart failure readmission rate fell well below the national average at 14.8 percent.

“Our heart failure nurses champion best practices by utilizing evidence-based guidelines and collaborative team-based approach that helps ensure the best possible outcome and consistency in patient care,” said Stephen Gunadi, pharmacist, Providence St. Peter Hospital. “As a result of the implementation of our transition-of-care program, the heart failure population at the hospital receives comprehensive care when admitted and discharged. In keeping with the continuity-of-care for our patients, important information regarding discharge medications is also passed on to the patient’s primary care physician.”

The successful pilot of this project ensured  thoroughly reviewing all of a patient’s home medications at admission, ensuring patients are on appropriate medications, thoroughly reviewing all patient’s discharge medications, filling prescriptions for patients prior to discharge, and providing bed-side delivery of these medications along with extensive patient education regarding medication therapy and lifestyle modification.

Winners of the 2016 Washington Awards of Excellence in Healthcare Quality in Washington are selected by a panel of expert Washington stakeholders. Award recipients were recognized during a ceremony at the 2016 Northwest Patient Safety Conference on Wednesday, May 4 in Lynnwood.
